Internal Bra Techniques & Auto-Augmentation

Internal mesh sling. A biocompatible mesh sling is placed under the lower portion of the breast to provide internal support to the breast tissue or implants. This sling is gradually absorbed by the body in a process known as hydrolysis and replaced by firmer, more durable natural breast tissue. An internal bra gives extra support to the breast tissue and promotes a longer-lasting outcome, particularly when the breast lift is combined with breast implant augmentation.

During breast lift surgery, Dr. Ravi meticulously places a series of internal sutures in the breast tissue to provide extra lift and support. Dr. Ravi performs an Auto-Augmentation by taking the native breast tissue (If there is enough), re-orients the breast tissue, and adds advanced pedicle shaping suturing techniques to make it look like an implant; then he will do a breast lift around that. This helps those patients who want more upper pole fullness who are reluctant to have implants placed. He may also use the internal bra (the internal mesh sling).

The Breast Lift Pre-Op Evaluation:

All mastopexy patients will be seen in-office, approximately two weeks before their breast lift procedure. At this time, “before” photographs are taken of the breasts. A detailed information packet will be provided, with a clear list of “dos” and “don’ts,” and any last-minute questions will be answered. Dr. Ravi will write prescriptions for an antibiotic, pain medication, and anti-nausea medication, giving you plenty of time to have them filled before surgery. Finally, all surgical consent forms will be signed.

Breast lift patients need to avoid all aspirin and nonsteroidal anti-inflammatory medications (NSAIDS) for at least two weeks before their surgery and two weeks after. This can prevent excessive bruising or unexpected bleeding. Additionally, smoking and nicotine products should be stopped at least four weeks prior to surgery and four weeks after surgery. This is vital in preventing complications and delayed wound healing.

Dr. Ravi’s Breast Lift Procedure

Dr. Ravi has his own QuadA surgery center (Houston Plastic Surgery Center) that is next to our clinic and he does all his cases there. Depending on the specific technique and the extent of correction, the surgery will take from two to three hours. The mastopexy is performed as an outpatient procedure using general anesthesia under the care of a board-certified anesthesiologist.

Breast Lift Incision Options

During the procedure, Dr. Ravi determines the best incision based on your tailored treatment plan. The type of incisions may include the following:

Circular, Periareolar Incision (Donut Mastopexy or Benelli Mastopexy)

This incision circles around the areola only, providing a smaller, reshaped areola complex with tighter skin and a lifted breast.

Lollipop Incision (Circumvertical Short-Scar Mastopexy)

The lollipop incision is created around the areola and then runs vertically downward, stopping at the breast crease.

Anchor Shape Incision (Wise Pattern Breast Lift)

This incision goes around the areola, down the center of the breast, and horizontally across the breast crease. For those women with excessive skin and tissue, the anchor-shaped incision provides the most aesthetically appealing results. The bottom portion of the scar is well hidden within the natural breast fold.

How Is a Breast Lift Procedure Performed?

To start, Dr. Ravi will create thin, predetermined incisions. From here, he will remove the stretched, lax skin, lifting the remaining breast tissue.

If needed, Dr. Ravi will reshape and reposition the nipple for a more natural-looking breast. Finally, Dr. Ravi will close the incisions with dissolvable sutures. A compression garment is put in place to reduce swelling and increase comfort, as well as to promote healing. The results of breast lift surgery are generally long-lasting, although this will depend on the individual’s skin elasticity and lifestyle.

Patients should expect their breast lift recovery to take about one week. For the first 24 hours after surgery, Dr. Ravi recommends having a responsible adult close at hand to assist with any needs that may arise. It is common to be tender and sore for the first several days, and some women note tightness across their chest. Dr. Ravi will prescribe the necessary medications for a comfortable recovery.

Women are normally able to get up and walk the same afternoon of surgery and, within 48 hours, and are generally able to perform light activities around the house. Most women are back to work in less than a week. However, heavy lifting and vigorous activities should be put off for three to five weeks.

Mild bruising can be expected as well as temporary numbness. Swelling and shape distortion may be seen initially, although this will dissipate over time. Residual swelling may last for up to six months. A surgical bra or compression garment will need to be worn for the first several weeks, keeping swelling to a minimum and encouraging proper healing.

Follow-ups will be scheduled with Dr. Ravi within the first week of your breast lift. Additionally, several appointments will be arranged over the first year so that Dr. Ravi can keep a close eye on your healing process.
